Feel free to download this blank order form or use your own. Mail order form along with check, made payable to ARCOFSF. If your organization is tax exempt, please include a copy of your tax exempt certificate. Please use the shipping chart below to determine your shipping cost. Florida sales tax is 6%.

To Remove an Item from the Shopping Cart:
Check the "Remove" selection box (to the right of "Quantity") and then click the "Update" button (located below the total line).
To Proceed to Checkout and Enter Payment Information:
Click on the "Checkout" button (located below the total line).
Please note: Once you click the "Checkout" button, you will be unable to edit quantities.
To Complete your Order:
Once you have completed the Payment, Billing & Shipping Information, click the "Checkout" button at the bottom of the page and you will be taken to the confirmation page.

Autism Resource Center of South Florida will provide the Products and/or Services ordered by the Client, subject to all of the terms, conditions and limitations applicable to the Services set forth on the website. The Client will pay for the Products or Services at the price indicated in the order. We may change the prices for the Products or Services at any time and any such change in prices will be effective immediately on this website. All fees collected under this Agreement are fully earned when due and nonrefundable when paid. All purchases due under this Agreement must be paid in United States dollars via credit card or other payment method acceptable during the Cart Checkout. Client agrees to provide its accurate name, mailing address, telephone and other contact information in Client’s profile and to update such contact details as they may change.
Client agrees that Autism Resource Center of South Florida may use data collected from the Client only as permitted by the Privacy Policy and we agreed to use such data only as permitted by such Privacy Policy, the terms of which are incorporated herein by this reference as if fully stated herein
